How do I become a patient?
Contact the office at (502) 657-4551 to discuss scheduling, deposit, and new patient paperwork.
Most patients are seen within two to three weeks.
Why do I need to fill out so much paperwork?
By providing so much information to Dr. Sullivan prior to the appointment, a patient’s needs can quickly be addressed. It allows efficient use of face-to-face time in order to develop a thorough treatment plan.
What will my first appointment be like? How should I prepare?
The first appointment will last approximately 60 to 70 minutes. At least 48 hours prior to the appointment, all intake paperwork needs to be submitted. Failure to complete paperwork may lead to the cancellation of the appointment and loss of deposit. Patients should arrive 5–10 minutes early for the appointment. Dr. Sullivan will review all submitted paperwork in order to become familiar with the patient’s history and current concerns. This will allow for maximum efficiency and planning during the initial appointment. The goal is to obtain a thorough history and devise an appropriate therapeutic plan during the initial meeting.
What is your payment policy?

Payment in full is due at the time of service.  Credit cards, HSA/FSA cards, personal checks and cash are accepted. There is a $50 charge for returned checks.

What insurance coverages do you accept?

We do not accept insurance but many insurance companies consider Dr. Sullivan an out of network provider. Our office can provide the appropriate paperwork so that the patient can file an insurance claim. Patients should contact their insurance company to inquire about reimbursement.

 

Dr. Sulllivan is NOT a Medicare or Medicaid provider.
How do you deal with cancellations, missed, or late appointments?
The patient must provide 24 hours notice of cancellation for all appointments. If not, they will be expected to pay a late cancellation fee (see financial agreement). The fee must be paid before rescheduling the appointment. Patients that arrive more than 10 minutes late to their appointment may need to be rescheduled and a late cancellation fee will be applied.
What should I do in case of an emergency?
If you are having a medical emergency or are having thoughts of harming yourself or someone else, you should call 911 or proceed to the nearest emergency room if you are able to do so safely. If you want to speak with a crisis counselor, you can call the Suicide/Crisis Hotline at 988. Once you are an established patient in our office, you will receive information on how to obtain assistance with urgent after hours needs.
What are your office hours?
Office hours are typically Tuesday, Wednesday, and Thursday 8:30 am – 4:30 pm.   Some new patient and emergency appointments are available Mondays and Fridays.
How do you handle medication refills?
Medications are refilled during regular office hours.  Urgent after hour refills MAY be filled, additional fees may apply (see financial agreement). The best way to have your medication refilled is to call our office.
